Band 5 Community Staff Nurse – Millthorpe District Nursing Team (Yealand Conyers)
About the Role
The Millthorpe District Nursing Team (now based at Yealand Conyers) within the Community, Cancer, Diagnostics & End of Life Division (CCDE) is seeking a full-time, permanent Band 5 Community Nurse.
We are looking for an enthusiastic and highly motivated professional who is eager to join an evolving district nursing service. Success in this role requires a commitment to collaborating with health and social care colleagues to deliver high-quality community care for patients across a range of medical conditions.
Key Expectations
- Work closely with patients, carers, health and social care teams to develop care programs.
- Undertake complex nursing assessments and communication-intensive responsibilities.
- Support the evolving 24-hour district nursing service, ensuring service continuity.

Care Delivery:
- Provide patients with a wide spectrum of health needs within their homes and community settings.
- Develop and implement tailored programmes of care, working collaboratively with multidisciplinary teams—including doctors, social workers, and informal carers.

- Engage closely with healthcare colleagues, voluntary organisations, and carers for holistic patient support.

Essential Requirements
- Must hold a full UK driving licence.
- Requires access to your ownVehicle to undertake home visits and community duties.
Economic & Location Context
The position operates across:
- Three main hospitals:
- Furness General Hospital (FGH), Barrow (with full Emergency Department and critical care facilities).
- Royal Lancaster Infirmary (RLI) (comprehensive acute and specialty services).
- Westmorland General Hospital (WGH), Kendal (Urgent Treatment Centre for non-life-threatening conditions, day surgeries, and diagnostics).
- Additional community facilities:
- Millom Hospital/GP Practice
- Queen Victoria Hospital, Morecambe
- Ulverston Community Health Centre
