An Exciting Opportunity in Adult Community Nursing
Homerton Healthcare NHS Foundation Trust is an integrated community and acute trust providing high quality service to residents of City and Hackney borough.
We Are Seeking a Band 5 Community Staff Nurse
We are looking for an enthusiastic Band 5 Community Staff Nurse to join the Clinical Assessment Team (CAT), based within the ACN Single Point of Access on a fixed term 12 month contract.
About the Role
The successful candidate will undertake holistic initial assessments on individuals referred to the ACN service. They will use their clinical skills and knowledge to implement care plans as well as to promote independence and self-care. They will work within a team that assesses patients and holds a caseload for short-term care interventions. They will liaise with the ACN Neighbourhood teams, GPs, social services, therapists, care agencies, and many other multidisciplinary teams. They will work under the guidance of the team’s Nurse Practitioners and Senior Nurse Practitioner.
