Community Staff Nurse – Band 5 District Nursing

Location: Hillingdon Community Services | Central and North West London NHS Foundation Trust (CNWL)

Contract Type: Full-Time / Community Nursing

Salary: NHS Band 5

Start Date: As soon as possible

About The Role

We are recruiting Band 5 Community Staff Nurses to join the District Nursing teams within CNWL Community Services in Hillingdon.

This is an excellent opportunity for nurses looking to develop their skills within a supportive and forward-thinking community nursing service, delivering high-quality care to patients in their own homes and community settings.

Key Responsibilities

As a Community Staff Nurse, you will:

  • Work both independently and within a multidisciplinary team to deliver safe, effective, and patient-centred care within the community.

Skills You Will Develop

You will develop expertise in:

  • Autonomous community nursing practice
  • Diabetic management, blood glucose monitoring, and insulin administration
  • Tissue viability and wound care, including:
    • Compression therapy
    • Doppler assessments
    • VAC dressings
    • Pressure ulcer management
    • Surgical wound care
  • Bladder and bowel care, including:
    • Catheterisation
    • Continence assessments
  • Intravenous medication administration and PICC/midline care
  • Palliative and end-of-life care, including:
    • Symptom management
    • Syringe drivers
  • Management of specialist drains including nephrostomies and chest drains

Additional Duties

You will also:

  • Maintain accurate clinical documentation and patient records
  • Support patients, carers, and families with education and ongoing care needs
  • Work collaboratively with specialist teams and wider community services
  • Contribute to safe discharge planning and hospital admission avoidance

What We’re Looking For

Essential

  • Registered General Nurse (RGN) with current NMC registration
  • Excellent written and verbal communication skills
  • Ability to work effectively within multidisciplinary teams
  • Strong clinical and organisational skills
  • Sound IT skills and accurate record-keeping
  • Understanding of risk management within community settings

Desirable

  • Evidence of relevant CPD
  • Previous experience within community or district nursing
