As a Community Support Worker in the Therapy Team
You'll play a vital role in our service. You will work closely with the multi-disciplinary team made up of clinicians such as Physiotherapists to provide ongoing clinical activities such as, rehabilitation, recovery and support with daily routines and personal care.

Key Responsibilities
- Undertake a range of delegated clinical, therapy & nursing led activities to support the service users on the Integrated Network Team caseload.
- Provide safe, patient centred and effective clinical activities in the community setting.
- Work within adults services to deliver a full range of healthcare activities.
- Work flexibly with other teams or services, both within the locality and Sirona wide when the need arises at short notice.
- Be able and willing to travel throughout the service delivery geographical area.

Working Hours
Full and part time hours available with a mixture of shifts - currently this service runs from 8am - 8pm - 7 days a week.
