Role Description
The Community Support Worker is a full-time hybrid role based in Carmarthen, with a combination of community visits, meetings at local service hubs, and some work from home for administrative tasks. Day-to-day responsibilities include providing practical and emotional support to individuals and families, helping them access appropriate community resources, and promoting independence and wellbeing. The role involves conducting needs assessments, planning and monitoring support, maintaining accurate case records, and collaborating with health, social care, and voluntary sector partners. The Community Support Worker will also facilitate community activities, signpost to relevant services, and respond to emerging needs in a timely and sensitive manner.

- Candidates should possess strong Interpersonal Skills and Community Support experience, with the ability to build trust and rapport with people from diverse backgrounds.
- Candidates should possess effective Communication skills, including clear verbal and written communication and active listening capabilities.
- Candidates should possess Case Management skills, including assessment, planning, monitoring, and documentation of support plans.
- Candidates should possess knowledge of Community Health and an understanding of local services, resources, and support networks.
- Relevant qualifications in health and social care, social work, psychology, or a related field are beneficial.
- Experience in community-based roles, outreach work, or support work within health or social care settings is advantageous.
- Ability to work independently, manage time effectively, and use digital tools for record-keeping and communication.
- A valid driver’s license and access to transport for community visits may be required, subject to organizational policy.
