Job Title: Support Worker – Domiciliary & Outreach

Company: Spring Forth Health Care Ltd

Location: Moreton, Wirral (field-based, travel across service areas)

Job Type: Full-time, Permanent

Hours: 37.5–40 hours per week, shift-based (includes some evenings and weekends)

Pay: £13.45–£15.00 per hour

DBS: Enhanced DBS with Adult Barring List required

About the Role

Spring Forth Health Care is looking for a reliable, compassionate Support Worker to join our domiciliary and outreach team in Wirral. You'll travel to clients in their own homes and communities, helping them live independently, safely, and with dignity. This role suits someone who enjoys variety, works well unsupervised, and is committed to person-centred care.

What you'll be doing:

  • Delivering personal care, medication support, and daily living assistance in clients' own homes
  • Supporting clients with outreach activities — appointments, shopping, community access, and social inclusion
  • Following individual care and support plans, and updating records accurately after each visit
  • Promoting client independence, choice, and dignity at all times
  • Reporting changes in client wellbeing, welfare concerns, or safeguarding issues promptly to your line manager
  • Working flexibly across a rota that may include early mornings, evenings, and weekends
  • Travelling between multiple client locations during a shift, managing your time effectively

What we're looking for:

  • Experience in a care, support work, or similar caring role (desirable but not essential — full training provided)
  • A caring, patient, and reliable approach with good communication skills
  • Sound understanding of safeguarding and person-centred care principles
  • Full UK driving licence and access to your own vehicle (essential for outreach and domiciliary visits)
  • Unrestricted right to work in the UK (we do not offer visa or skilled worker sponsorship)
  • Enhanced DBS (or willingness to apply)

Desirable:

  • NVQ/QCF Level 2 or 3 in Health & Social Care, or working towards
  • Previous experience in domiciliary care or community outreach
  • Familiarity with CQC-regulated environments
