About the Role
As a Complex Case Manager, you will be assigned to specific hospital inpatient areas and oversee a diverse caseload of patients—ranging from simple to highly complex—ensuring their journey from admission to discharge is effectively managed.
Close collaboration with Discharge Coordinators is essential, along with mentoring and leadership to support and expedite timely, safe discharges.
This role is critical to hospital flow, acting as the primary escalation point to streamline patient progress and discharge. Your responsibilities include:
- Ensuring patients who no longer meet NCTR (No Clinical Need to Remain in hospital) are discharged promptly.
- Reducing ** Length of Stay (LoS)** and decision-making delays to optimise patient outcomes.
In the Emergency Department (ED), you will develop and execute patient care plans to either facilitate safe early discharges (with community support) or initiate discharge planning immediately upon admission.
Key Responsibilities
- Provide expertise in drafting patient care plans for both Emergency Department and inpatient settings.
- Work across hospital and community settings to ensure seamless discharge pathways.
- Collaborate with multi-agency partners to align care transitions.
- Lead on identifying and overcoming barriers to timely and safe discharges.
- Liaise with transfer hubs, community services, and community teams to coordinate patient transitions.
- Promote admission prevention where possible, enhancing overall patient flow.
- Support the shift towards 7-day-a-week discharge planning with flexibility across multiple sites.

Ideal Candidate
You must be a qualified healthcare professional with:
- A strong background in hospital discharge (including virtual ward operations).
- Multi-agency experience, with a proven track record in supporting patients to transition home.
- Action-oriented, capable of driving improvements and adapting to a rapidly evolving service.
- Commitment to working autonomously with stakeholders across hospital and community settings.
- Flexibility to travel and operate in a dynamic environment with three main hospitals and community sites:
- Furness General Hospital (FGH) – Barrow
- Royal Lancaster Infirmary (RLI) – Lancaster (including ED, Critical/Coronary Care, Consultant-led services)
- Westmorland General Hospital (WGH) – Kendal (Urgent Treatment Centre for non-life-threatening conditions)
- Community sites: Queen Victoria Hospital (Morecambe), Ulverston Community Health Centre, Millom Hospital, GP Practices
- Experience aligning with community health services (incl. home visits, clinics, care advice).
