Consultant Nurse/Palliative Care Specialist – Saint Francis Hospice

About the Role
You will join the palliative care team at Saint Francis Hospice, delivering consultant-level expertise to both the hospice ward team and community palliative care services in outer East London, Brentwood, Barking & Dagenham, Havering, and surrounding areas. The position is fully seconded to the hospice and community day-to-day, with clinical cooperation through a joint regional consultant on-call rota with Barking Havering and Redbridge University Hospitals NHS Trust (BHRUT).
Key Responsibilities
1. Hospice Ward Leadership
- Provide consultant-level clinical input to the ward team and multidisciplinary team (MDT), partnering with a fellow consultant.
- Lead and support medical trainees (post-graduate) and ward staff in delivering a holistic model of palliative care for patients with complex or advanced life-limiting conditions.
- Act as a key member of the weekly ward multidisciplinary team (MDT) meetings, ensuring evidence-based and patient-centric care.
2. Community Palliative Care Support
- Deliver consultant-level input to the hospice’s community team, with a clinical and strategic focus on one of the boroughs served (e.g., Barking & Dagenham, Havering, or Brentwood).
- Provide direct in-home visits for patients with high palliative care complexity.
- Offer indirect support through:
- Clinical advice and case reviews
- Education for medical teams and care providers
- Guidance on quality improvement initiatives in palliative care.
- Ensure continuity across secondary community care in partnership with primary sector colleagues and social care professionals.

3. Education & Research
- Contribute to staff training and medical education, including:
- Supporting medical trainees/learners (e.g., foundation doctors, speciality registrars, GP trainees).
- Facilitating learning in care pathways, education events, and quality improvement projects.
- Help establish research-active status for the hospice, embedding palliative care research into practice.
4. Regional & Out-of-Hours Commitments
- Actively participate in the regional consultant on-call rota, covering:
- Out-of-hours emergencies at BHRUT hospitals.
- Urgent palliative care cases within the hospice and community settings.
