The Critical Care Outreach Team (CCOT) is delighted to offer an exciting opportunity for a registered nurse with recent B6 experience in critical care or an emergency care background to join our team.
About the Role
CCOT is a 24/7 team supporting the Trust to safely and effectively manage our deteriorating patients.
Responsibilities
The Critical Care Outreach Practitioner is mainly responsible for:
- Rapidly assess and manage deteriorating patients across the hospital
- Initiating early interventions to prevent further decline
- Supporting safe discharge and follow-up of patients transitioned from Critical Care to ward environments
- Responding to urgent referrals and escalation calls, providing expert clinical advice and hands-on support
- Working collaboratively with multidisciplinary teams to ensure safe, coordinated patient care and appropriate escalation planning
- Educating and supporting ward staff in recognising and managing acute deterioration
- Contributing to patient safety, quality improvement, audit, and clinical governance processes
- Maintaining clear documentation and communicating effectively with patients, families, and staff
- Providing clinical leadership and support ongoing professional development within the team
