- ÂŁ15.79 per 1:1 coaching session
- Mileage: Paid
- Hours:
- Tuesday: 12:00–18:00
- Wednesday: 09:00–17:00
- Friday: 09:00–17:00
About the Role
As a Diabetes Practitioner, you will support individuals enrolled in our Type 2 Diabetes Remission Programme, helping them reduce blood sugar levels and achieve improved long-term health outcomes over a one-year programme.
Your responsibilities will include:
- Delivering personalised one-to-one coaching sessions via telephone and digital platforms
- Motivating and guiding participants to make sustainable lifestyle changes
- Helping set achievable goals and maintain engagement throughout their journey
- Providing ongoing support in overcoming barriers and building confidence
- Collaborating with colleagues to ensure high-quality outcomes for service users
Key Responsibilities
- Design and deliver personalised health coaching tailored to each individual’s needs
- Encourage behavioural and lifestyle modifications for remission and improved wellbeing
- Monitor progress and adjust support plans as necessary
- Foster trusting, compassionate relationships with service users
- Work alongside the Xyla Health & Wellbeing team to enhance service delivery

Requirements
Qualifications & Experience
- Minimum of an undergraduate degree in:
- Nutrition
- Health-related sciences (e.g., Sports/Exercise Science, Health Psychology)
- OR relevant experience in personal training with a Level 2/3 PT qualification
- Phone coaching experience and proficiency in:
- Digital platforms
- Microsoft applications and healthcare systems
Preferred Qualifications
- Relevant health/wellbeing credentials such as:
- Degrees/diplomas in Nutrition, Dietetics, Health Psychology or Sport Science
- CIMSPA fitness qualifications (Level 3+)
