Job Overview
This is an exceptional opportunity for an experienced and motivated Senior Enhanced Practitioner to join our rapidly expanding multidisciplinary Diabetes service.
You will work within a forward-thinking and supportive team delivering outstanding care across a wide range of specialist inpatient and outpatient services.
The successful candidate will utilise advanced clinical skills across antenatal diabetes care, insulin pump therapy, young persons and transitional services, acute diabetes management, complex diabetes clinics and specialist renal diabetes services.
This role is ideal for an experienced diabetes clinician who is passionate about delivering high-quality care and thrives in a dynamic clinical environment. You will manage complex patient caseloads, make advanced clinical decisions, provide education to patients and healthcare professionals, and work collaboratively across multidisciplinary teams.
You will play a key leadership role, contribute to education, quality improvement, and research activities.

Main Duties of the Job
- Provide expert, autonomous assessment, diagnosis and management of patients with complex diabetes needs across inpatient and outpatient settings.
- Deliver specialist diabetes care within dedicated clinics including antenatal, insulin pump, young persons, renal and complex diabetes services.
- Develop, implement and evaluate individualised care plans, promoting self-management and positive patient outcomes.
- Provide specialist diabetes advice, education and support to patients and their carers, and healthcare professionals across the Trust and wider healthcare community.
- Initiate and adjust diabetes treatments and interventions in line with clinical guidelines and scope of practice, including prescribing where appropriate.

Detailed Job Description And Main Responsibilities

- Assess, differentially diagnose, plan, implement and evaluate individual treatment interventions for diabetes patients by virtue of enhanced clinical judgement and close liaison with members of the MDT.
- Use diabetes expertise and decision-making skills to inform clinical reasoning approaches when dealing with individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and / or diagnoses.
- Initiate, evaluate, and modify a range of diabetes interventions which may include prescribing medicines, therapies, lifestyle advice and care. Use empowerment and counselling skills to enable individuals with diabetes to make life changes in complex situations and self-manage their condition.
- Provide specialist and expert diabetes clinical knowledge to colleagues, patients and carers/ relatives both within and outside the Trust.
- Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families, and carers.
- Ensure compliance with their respective code of professional conduct and work within their scope of practice.
- Works in conjunction with the MDT to clearly define values, direction and policies impacting upon care.
- Work in partnership with individuals with diabetes, their families, and carers, using a range of assessment methods as appropriate.
- Be able to demonstrate a critical understanding of their broadened level of responsibility and autonomy. This includes being critically aware of their limits of their own competence and professional scope of practice, including when working with complexity, risk, uncertainty, and incomplete information.
- Be responsible and accountable for their decisions, actions, and omissions at this enhanced level of practice.
- Utilise professional judgement and maturity, understanding when to seek help.
- Demonstrate critical reflection on own practice, self-awareness, emotional intelligence, and openness to change.
- Demonstrate effective communication skills, supporting people in making decisions, planning care, or seeking to make positive changes, using Health Education England’s framework to promote person-centred approaches in health and care.
- Work collaboratively with an appropriate range of multi-agency and inter-professional resources, developing, maintaining, and evaluating links to manage risk and issues across organisations and settings.
- Evidence the underpinning diabetes specific competencies i.e. knowledge, skills and behaviours relevant to the role setting and scope, and demonstrate application of the capabilities to these, in an approach that is appropriate to the individual role, setting and scope.
- Act as a clinical role model/ advocate for developing and delivering care that is responsive to changing requirements, informed by an understanding of local population health needs, agencies, and networks.
- Initiate drug therapy/ medication within the parameters of agreed clinical guidelines and in accordance with the legal framework you work under to supply and prescribe medicines (if from professional group legally entitled to prescribe).
- Request diagnostic intervention and investigation and interpret said investigation within the practitioner’s scope of practice.

Education/Training/Qualifications
Essential criteria
- Active Professional Registered Qualification with a professional body e.g., NMC, HCPC
- Post registration qualifications/CPD in area of diabetes practice as a minimum.
- Non-medical prescriber (If appropriate for statutory profession and role)
- Proven leadership experience
- Evidence of ongoing professional development through maintenance of professional portfolio and registration revalidation
Desirable criteria
- Enhanced Clinical Practitioner Level 6 or above qualification
Work Experience
Essential criteria
- Grounded knowledge of the diabetes environment
- Demonstrable experience of service improvement and transformation, achieving cultural change to improve service quality
- Experience of implementing local/specialist objectives
- Proven track record of successful management of change and leadership
- Excellent organisation and leadership skills
- Ability to act independently, resolve complex clinical situations and have good decision-making skills
- Experience of management of the complex diabetes patient
- Experience of involvement in research and audit at a local level within current or previous roles.
- Ability to interpret and apply best practice and promote its use with others
- A sound understanding of professional priorities and issues
