Dietetic Family Support Worker (Assistant Practitioner)
Family Support Worker – Anglesey Health and Wellbeing Pilot Programme
About the Role
THIS POST IS FIXED TERM/SECONDMENT UNTIL 31 MARCH 2027 DUE TO FUNDING.

A unique opportunity has arisen within a pilot programme on Anglesey, supporting the health and wellbeing of children and families.
The successful candidate will work alongside family support workers under the guidance of the Senior Project Coordinator, delivering and expanding support services to families. This includes:
- Engaging with families in their homes and delivering community-based interventions
- Encouraging positive changes in food, nutrition, and physical activity behaviours using agreed approaches
The role involves:
- Enhancing the project’s reach through Whole System Approach partnerships
- Operating within an agreed framework with autonomy, responsibility, and accountability for caseloads, group support, and collaboration with partners/organisations in a complex community setting.
- Supporting the Senior Project Lead in monitoring, quality-assuring interventions, and evaluating sustainability of approaches.
Key Responsibilities
Delivering nutrition support
- Plan, coordinate, and deliver 1:1 and group nutrition education sessions (e.g., practical cooking skills, community-based nutrition).
- Work under the direction of the Project Dietitian and Senior Project Lead.

Engaging with families
- Motivate, guide, and encourage family participation in health-promoting activities.
- Monitor progress, maintain accurate records, and report outcomes and findings to the team/partners.
Initiative and autonomy
- Operate independently on agreed project areas under guidance from the Senior Project Lead.
- Seek advice when necessary while maintaining accountability.
Building partnerships
- Develop and maintain positive stakeholder relationships (e.g., local authority, health workers, leisure staff, voluntary sector).
- Attend partnership discussions to share best practices and reflect on success areas.
Resource creation
- Produce training materials such as PowerPoint presentations, informational sheets, displays, handouts, and evaluation tools.
Language
The ability to speak Welsh is desirable; however, English and/or Welsh speakers are equally welcome to apply.

Qualifications
Essential Criteria
- Good general education (evidence of sound English language skills and mathematics).
- Relevant Level 3 qualification in Healthcare or short courses/equivalent experience.
- Willingness to pursue a Level 4 credential within 3 years of the post.
Desirable Criteria
- Degree in related fields (e.g., Nutrition, Health Science).
- Food Hygiene Certificate.
Experience
Essential Criteria
- Experience in the NHS, social care, or community settings.
- IT proficiency (Microsoft Office, Teams, Outlook).
- Experience working with children/families or relevant community groups.
Desirable Criteria
- Experience presenting to small groups.
- Attendance/relevance to evidence-based further training in related areas.
Skills
Essential Criteria
- Ability to work independently or as part of a team with families/community groups.
- Excellent verbal/written communication skills.
Desirable Criteria
- Fluency in Welsh (though not mandatory).
