Directorate Manager (Integrated Community Services)

About the Role

We have well established Integrated Community Services in Bridgend delivered in partnership between Cwm Taf Morgannwg UHB and Bridgend CBC. Our multi-disciplinary services work closely with primary care, health and social care services and community partners to deliver the best health and wellbeing outcomes. This is a new senior role to build on our existing model and lead our community services into the future.

We are seeking an experienced health and or social care professional to lead and drive our vision and continue the transformation and modernisation of integrated health and social care services.

We wish to make a joint appointment for a compassionate and collaborative leader who is committed to delivering high quality outcomes. This is a pivotal role responsible for operational management, shaping and delivering high-quality, person-centred integrated services for older people and individuals living with long-term and chronic conditions. The post holder will work collaboratively across organisational boundaries to deliver seamless, integrated care that meets the needs of local communities. Being equally passionate in enhancing staff experience and wellbeing and able to influence at different levels within our organisations.

Responsibilities

  • Have operational responsibility for the performance of community services within the county, contributing to the development and preparation of plans for the achievement of national and local priorities.
  • Provide leadership and management combined with operational responsibility for the resources and staff within the community services for the county.
  • Enable effective delivery of services which reflect local community and stakeholder influence and the Health Board’s and Council’s vision, supported by staff located in the community setting.
  • Ensure that Health Board and the Council aims and objectives and County targets and strategies are clearly communicated to all community-based staff so that a full contribution can be made to implementation of operational plans reflecting these.
  • Direct, lead and motivate staff within the community to ensure a high standard of professionalism, efficiency and effectiveness in service delivery, ensuring that activity is aligned to the Health Board and the Council plans and strategies.
  • Undertake coaching, mentoring, resource planning, standard setting, performance management, team and individual development to ensure a culture of continuous improvement and professional excellence is achieved.
