Discharge Coordinator
**Discharge Coordinator (Complex Discharge)
** Role Summary
Liaise internally with multidisciplinary teams, senior staff, ward staff, and Allied Health Professionals. Liaise externally with Social Services, Primary Care Services, General Practitioners, Housing, Homeless Persons Unit, Department of Health, Borders Agency, and the Office of the Public Guardian.
- Promote a smooth discharge process by focusing on tasks that support the multidisciplinary team in ensuring safe and timely discharge of patients.
- Complete discharge planning documentation.
- Communicate with external agencies (e.g., Social Services), patient families, and carers.
- Arrange appropriate transport.
- Liaise with all Multidisciplinary Team members, including but not limited to:
- Occupational therapy
- Physiotherapy
- Speech and Language Therapy
- Dietetics
- Clinical Psychology
- Medicine

Provide access to patient homes for external agencies, accompany patients to their homes (as appropriate), or assist with attending appointments with external agencies.

Monitor and follow-up with multi-disciplinary team members and external agencies to ensure timely and safe discharge, optimise bed usage, and reduce length of stay.

Handle complex discharge issues.

Ensure Discharge Notifications to Social Services are issued responsibly, observing reimbursement guidelines.
