Discharge Process Facilitator – Band 3

Location: UNHT (North Tees & Hartlepool NHS Foundation Trust)

Join our Integrated Discharge Team and Enhanced Care Team to provide an efficient, effective, and comprehensive administrative support for seamless patient flow and care transitions.


Key Responsibilities

  • Assist in smooth patient discharges, working closely with:
    • Ward staff
    • Therapies teams
    • Patient flow team
    • External partners (social care, voluntary agencies)
  • Facilitate and optimise discharge processes while identifying opportunities to meet key performance indicators (KPIs)
  • Coordinate transport scheduling to ensure timely movements for patients
  • Produce high-quality work outputs, ensuring compliance with:
    • NHS Patient Safety guidelines
    • Information Governance protocols
    • CQC recommendations
    • Trust policies, procedures, and guidelines
  • Utilise appropriate technologies and equipment to streamline administrative tasks

About the Role

We are looking for a flexible, adaptable, and team-oriented professional who:

  • Has excellent communication skills and can comfortably work across multidisciplinary boundaries
  • Can provide whole-person care management for patients in transition
  • Is independent yet collaborative, able to work well in teams and autonomously
  • Thrive in a dynamic environment with varying diary tasks

This opportunity is part of Patient Flow, focusing on administration and patient mobility—crucial roles in ensuring safe, efficient, and compassionate care.


Person Specification

  • Communication: Strong interpersonal skills with ability to liaise across professional boundaries
  • Teamwork: Committed to a collaborative approach in a fast-paced setting
  • Initiative: Ability to identify bottlenecks in discharge processes and propose solutions
  • Attention to detail: Ensures accuracy in documentation and compliance with NHS standards
  • Digital literacy: Comfortable using ** NVS, electronic patient records (e.g. SystmOne), and scheduling tools**
