Discharge Support Worker (Floating Support Team - Mental Health) – Part-Time
£13.50 per hour Part-Time (Max 32.5 hours) Greater Manchester
About the Role
We are seeking a Discharge Support Worker to join our Mental Health Floating Support Team, operating within wards and community settings across North and South Manchester.
This highly-responsive role delivers hospital in-reach services to support individuals with mental health needs and additional complexities (such ashomelessness, financial difficulties, or criminal history) that may hinder discharge. You will provide person-centred assistance to help users achieve stable housing, financial wellness, and meaningful pathways in areas like independent living, social inclusion, and employment.
Key Responsibilities
- Conduct person-centred assessments to identify and address housing and support needs
- Set and track clear targets, outcomes, and quality standards
- Offer direct support and practical assistance in securing housing and accessing support services
- Help users manage financial resources to improve financial well-being
- Forge partnerships with housing providers and support networks to create sustainable solutions
- Facilitate smooth transitions from hospital to community-based care

Requirements
- Essential: Proven experience in supporting individuals with mental health needs
