District Nurse Planned Care Nursing East/West
Job Title
We are seeking a motivated and compassionate District Nurse to join our community team. This is a rewarding opportunity to make a real difference by delivering high-quality care to patients in their own homes.
Essential Criteria
- District Nursing qualification (DN SPQ)
Key Responsibilities
- Assess patients in their homes, developing, implementing, and evaluating individualised care plans
- Manage a caseload with the support of the Clinical Lead
- Provide leadership and line management to a team of Registered Nurses and Healthcare Assistants
- Work flexibly across teams to meet changing service demands and ensure continuity of care
Our core service operates 8:00am – 6:00pm, 7 days per week, and flexibility is essential to support the needs of the service.
This role requires a professional who demonstrates a positive attitude, compassion, resilience, and vigilance, ensuring safe and effective patient-centred care at all times.

- A District Nursing qualification (DN SPQ) is an essential criteria.
- Assessing new patients for nursing care in their own homes, creating and evaluating care plans.
- Managing a caseload with support of the Clinical Lead
- Line Management and leadership of a team of Registered Nurses & HCAs.
- Our core service hours are 08:00 to 18:00, 7 days per week.
