Are you ready to lead innovation and make a real difference in District Nursing?

Join us as a key driver in delivering the Ageing Well Anticipatory Care Model, working hand-in-hand with senior leaders to transform care for local populations.

In This Exciting Role, You’ll

  • Lead with Impact: Provide clinical leadership and champion best practice across District Nursing teams, ensuring patients are aligned to correct care pathways
  • Drive Excellence: Ensure planned care meets the highest quality standards and evidence-based pathways, delivering outstanding outcomes for patients.
  • Empower Teams: Guide and support District Nursing teams in managing complex and advanced patient needs.
  • Collaborate for Success: Build strong relationships and integrated working with multi-professional teams to bring the national model to life locally.

You’ll work closely with the District Nurse Locality Leads, offering strategic leadership and hands-on support to create a seamless, patient-focused service.

Requirements

Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.

Role Description

As a District Nursing Lead (Advanced District Nurse Specialist), you will provide expert clinical leadership and proactive management of District Nursing teams, ensuring high-quality, evidence-based care for patients with complex needs.

You’ll lead the implementation of the national anticipatory care model, champion preventative and personalised care, and work collaboratively with GPs, multi-professional teams, and locality leads to reduce hospital admissions. Your role includes advanced clinical assessment, prescribing, and developing care pathways, alongside supervising and supporting staff to deliver safe, effective care.

To succeed, you’ll need strong clinical leadership skills, advanced assessment and decision-making abilities, and a commitment to continuous improvement. Excellent communication, emotional intelligence, and the ability to manage competing priorities under pressure are essential, as is experience in change management, education, and integrated working.

You’ll be a role model for professional standards, fostering a culture of learning and quality across the service.
