Job Description:
Job Title: Day Domiciliary Health Care Assistant
Location: London Borough of Hammersmith & Fulham
Pay: £12.21 – £15.00 per hour
Job Type: Permanent | Full-Time & Part-Time

What You’ll Be Doing:
- Supporting individuals with daily living tasks including personal care, meal preparation, and medication
- Promoting independence and empowering individuals to make their own choices
- Assisting with household tasks and maintaining a safe, clean environment
- Supporting service users to access community and social activities
- Upholding safeguarding, health & safety, and well-being standards
- Building strong, positive relationships and maintaining accurate care records

What We’re Looking For:
- A kind, empathetic nature and genuine passion for helping others
- Strong communication skills and a collaborative mindset
- Flexibility to work a variety of shifts, including evenings and weekends
- Minimum 6 months’ experience supporting individuals with autism or learning disabilities
- Level 3 Diploma in Health & Social Care (or working towards it)
- Training in Autism Awareness and/or Positive Behaviour Support (PBS) preferred

Additional Information:
- A full enhanced DBS check is required (we will support you with this)
- Must provide 3 years of references
- Attendance at a 3-day paid induction and a competency-based interview is required
