- Salary and hours: £37,509–38,637 per annum, 36 hours per week

- Contract: Temporary until 31 March 2029

About The Role
The Hammersmith & Fulham Family Hubs Team is seeking an Early Years Practitioner to deliver a range of supportive activities in family hubs and provide outreach services.
This critical role focuses on laying foundational mental health, emotional regulation, and learning foundations for young children while building parenting resilience. The successful candidate will guide families to achieve positive outcomes for both children and parents through tailored support.
Responsibilities include:
- Providing early years support within family hubs and linked children’s centres
- Delivering the Children’s Centre core offer and universal family support services
- Travelling across the borough to deliver integrated services, including SEND (Special Educational Needs and Disabilities) support
- Ensuring compliance with statutory requirements, national guidelines, and performance targets
- Guiding families in behaviour, learning, and childcare while supporting outreach activities

Job Responsibilities
- Support delivery of early years provision in family hubs and childcare centres
- Collaborate with partners to fulfil universal and extended services
- Expand early years services and outreach initiatives within established frameworks
- Uphold EYFS (Early Years Foundation Stage) Curriculum Guidance for all young children, including those with SEND
- Offer families guidance, advice, and social-emotional support
- Work closely with specialist SEND teams to integrate assistance into family hub services
- Undertake reception duties and alternative roles as required
Qualifications & Requirements
- Qualified Early Years Practitioner (minimum Level 3)
- Enhanced DBS disclosure essential
- Ability to travel between sites across the borough
