EIP Care Coordinator

Registered Mental Health Nurse – Band 6 Care Coordinator (Early Intervention Psychosis Service – EIP)

About Bradford Early Intervention Service

We are dedicated to delivering high-quality care for individuals across Bradford, Airedale and Craven. Our mission is rooted in respect, collaboration, and student-centred support through a bio-psychosocial, spiritual, and recovery-based approach.

We are seeking a Registered Mental Health Nurse (RMN) to join our vibrant Early Intervention Psychosis (EIP) team as a Band 6 Care Coordinator. This role forms a key part of our multidisciplinary approach, enabling us to make a transformative impact on the lives of people experiencing their first episode of psychosis, along with their families and supporters.

The Role

The Care Coordinator plays a central role in assessing, planning, and delivering personalised care packages to individuals and families affected by psychosis. Working in close partnership with service users, carers, and partner agencies—including social care, GP practices, and allied health professionals—you will:

  • Assess, plan, and implement care coordinations while adhering to CPA (Care Programme Approach) standards.
  • Provide regular supervision and mentorship to junior staff within the team.
  • Support the team depot clinic, ensuring smooth referrals, medication management, and wraparound care.
  • Utilise a psycho-educational and recovery-focused approach, including:
    • CBT (Cognitive Behavioural Therapy) skills
    • Family interventions and coordination
    • Physical health monitoring and advocacy
    • Vocational/rehabilitation interventions
    • Living skills training tailored to the individual’s needs
  • Work closely with individuals and families to promote spiritual and psychosocial well-being, emphasising independence and a positive long-term outlook.
  • Collaborate with the multidisciplinary team, drawing upon the expertise of crisis workers, peer support workers, occupational therapists, and psychiatrists.
  • Coordinating follow-ups across real-time mental health services, ensuring continuity of care**: as individuals transition into long-term support.

Skills Requirement

  • Post-registration RMN qualification, with current registration on the Nursing and Midwifery Council (NMC) Register
  • A minimum of 2 years’ post-registration experience, preferably within mental health nursing or psychosis care environments.
  • Demonstrated experience in delivering complex mental healthcare, often within early intervention or crisis intervention frameworks.
  • Proven mentoring and supervisory skills, with experience of supporting fellow mental health professionals.
  • Commitment and ability to travel across Bradford District regularly.
  • Evidence of understanding and alignment with outcomes-based care pathways.
  • Knowledge of the role of NICE best practice guidance in mental health provision.
