CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

The purpose of the EIV pathway is to deliver crisis management to patients living in Herts Valleys and to prevent a hospital admission, providing a clinical assessment by a nurse or paramedic jointly with a HCA. The Unplanned Care team operates from 08:00 to 22:00, 7 days a week. The newly established EIV will supplement and support the delivery of Rapid Response and Discharge to Assess in the Unplanned care service.

Apply knowledge of the Mental Capacity Act and Care Act. Provide evidence-based clinical/therapeutic interventions based on 'best practice', in order to improve health outcomes and promote choice. Work in close collaboration with all other community agencies, including Locality planned care teams, In-Patient services and GPs.
