About the role
We believe everyone should have access to high-quality, evidence-based support to improve their health. The Primary Care Engagement Lead plays a vital role in making that happen.
Working closely with GP practices, Primary Care Networks and Integrated Care Boards (ICBs), you'll help ensure healthcare professionals understand our programmes, know which patients are eligible and feel confident referring them. By increasing both the quality and volume of referrals, you'll directly support more people to access life-changing interventions while helping reduce health inequalities across your region.
This is a highly visible role where relationship building, data-driven decision making and a passion for improving patient outcomes come together to make a measurable difference.
As a Primary Care Engagement Lead, you'll become the trusted local expert for our programmes, supporting healthcare professionals and helping them maximise opportunities to improve patient outcomes.
What You'll be Doing
- Build strong relationships with GP practices and primary care teams to increase awareness of our programmes and encourage appropriate patient referrals.
- Educate and support healthcare professionals by delivering presentations, training sessions and workshops, giving them the confidence to identify and refer eligible patients.
- Use referral and outcome data to identify trends, opportunities and areas requiring additional support, helping shape local engagement plans.
- Develop targeted engagement strategies alongside the Key Account Manager to improve referral quality, increase uptake and achieve agreed performance targets.
- Champion equitable access by identifying practices and communities that are currently underrepresented and implementing tailored engagement approaches.
- Represent Momenta with commissioners and ICB stakeholders, presenting referral and outcome data and helping demonstrate the impact of our programmes.
- Provide ongoing support to practices by answering queries, resolving challenges and making the referral process as straightforward as possible.
- Work collaboratively across internal teams, ensuring referral processing, onboarding and communications are accurate, timely and deliver an excellent experience for both practices and patients.

You’ll have
- We're looking for someone who combines excellent relationship-building skills with a genuine passion for improving healthcare.
- An in-depth knowledge of the programme, with current experience of delivering it to participants and/or engaging healthcare professionals about the programme.
- Experience building trusted relationships with healthcare professionals, ideally within primary care.
- Excellent communication and presentation skills, with the confidence to influence and educate a range of audiences.
- A target-focused approach and experience delivering measurable outcomes.
- An understanding of primary care, referral pathways and the wider NHS landscape.
- Strong analytical skills with the ability to interpret data and turn insight into action.
- Excellent organisational skills and the ability to manage multiple priorities independently.
- Experience using Microsoft Office and CRM systems.
- It would also be beneficial if you have experience within public health, behaviour change or lifestyle services and an understanding of Integrated Care Boards and wider NHS structures.
