We have an exciting opportunity for an enthusiastic individual with the skills, drive, and commitment to join our service as an Enhanced Care Specialist Practitioner/Team Lead and make a real difference to the people we care for.
About the Role
This clinical leadership role combines specialist clinical practice with operational leadership of the Enhanced Care and Support Team, including participation in a 7-day rota (8am–8pm).
The post holder ensures safe, effective and consistent care for patients requiring enhanced observation and therapeutic support, including:
- People with mental health needs
- People living with dementia
- People experiencing delirium
- People with learning disabilities
- Autistic people
- People with eating disorders
Key Responsibilities
Clinical Leadership & Care Coordination
- Work closely with matrons, multidisciplinary teams, and specialist leads to standardise enhanced care processes in line with professional and statutory guidance
- Undertake holistic assessments
- Support risk management and care planning
- Coordinate resources and staffing to ensure safe staffing and high-quality care
Expert Clinical Advice & Education
- Provide expert clinical advice, leadership and education to support a skilled and competent team
- Contribute to governance, audit and service improvement to promote a culture of learning and excellence
- Build partnerships with ward teams and the multidisciplinary team to assess, plan, and review care, including managing risk and supporting complex decision-making

Specialised Clinical Practice
- Deliver expert clinical practice including:
- Holistic assessments
- Triage
- Care planning
- Provide specialist advice on:
- Mental health
- Legal frameworks
- Person-centred care to support safe and effective practice
Staff Development & Supervision
- Support and develop staff through supervision, coaching, training, and performance management
- Act as line manager for Enhanced Care Practitioners
Quality & Service Improvement
- Drive quality improvement by contributing to:
- Audit
- Governance
- Policy development
- Service development
- Monitor standards of care, respond to incidents or concerns, and uphold safety and quality
- Encourage evidence-based practice, contributing to:
- Research
- Audit
- Dissemination of learning to improve patient outcomes
Effective Communication
- Ensure effective communication with:
- Patients
- Families
- Carers
- Teams
- Partner agencies
