Enhanced Care Specialist Practitioner/Team Lead

We have an exciting opportunity for an enthusiastic individual to join our service as an Enhanced Care Specialist Practitioner/Team Lead and help deliver high-quality, person-centred care.

About the Role

The role combines:

  • Specialist clinical practice
  • Operational leadership of the Enhanced Care and Support Team
  • Participation in a 7-day rota (8am–8pm)

The post holder: ensures safe, effective, and consistent care for patients requiring enhanced observation and therapeutic support, including those with:

  • Mental health needs
  • Dementia
  • Delirium
  • Learning disabilities
  • Autistic individuals
  • Eating disorders

Key Responsibilities

Clinical Leadership & Care Coordination

  • Work closely with matrons, multidisciplinary teams, and specialist leads
  • Lead the standardisation of Enhanced Care Processes in line with professional/statutory guidance
  • Undertake holistic assessments, support risk management, and care planning
  • Coordinate resources, staffing, and high-quality care

Clinical Practice & Support

  • Provide expert clinical advice, leadership, and education to the Enhanced Care Team
  • Deliver holistic assessments, triage, and care planning
  • Offer specialist advice on:
    • Mental health
    • Legal frameworks
    • Person-centred care
  • Monitor standards of care, responding to incidents and ensuring patient safety

Team Development

  • Supervise, coach, and train Enhanced Care Practitioners
  • Drive a positive learning culture
  • Ensure staff competence and support personal career growth

Quality Improvement & Governance

  • Contribute to audit, governance, policy, and service improvement
  • Promote evidence-based practice through research and shared learning

Communication & Coordination

  • Work with patients, families, carers, teams, and partner agencies
  • Ensure coordinated and safe care

Professional Development & Advocacy

  • Champion compassionate, inclusive care aligned with Trust values
