Band 5 Experienced Community Staff Nurse – Placed Based Team 2

About the Role

Are you looking for a change of scenery? Would you like to work outdoors and provide nursing care in diverse environments daily? If so, consider this role.

We are seeking a Band 5 Experienced Registered Nurse to join our forward-thinking Community Nursing Team. Our mission is to deliver high-standard, evidence-based care to housebound patients within their homes, ensuring unnecessary hospital admissions are avoided.

Key aspects of the Place-Based Team (Team 2) include proactively managing home care and facilitating client independence while aligning with the standards of our Outstanding NHS Trust.

Successful candidates will:

  • Have a general knowledge of current best practice in Community Nursing.
  • Foster strong cross-professional collaboration across the Integrated Health Team.
  • Provide comprehensive support in patients’ homes and clinics.

Key Responsibilities

  • Act as a Community Staff Nurse in an Integrated Health Team role.
  • Deliver high-quality nursing care and health promotion across professional boundaries.
  • Adapt care to meet the unique needs of your local population.
  • Work collaboratively with colleagues to ensure compassionate and inclusive practices.

Requirements & Qualifications

  • Valid UK driving licence (travel across rural areas essential).
  • Experience in Community Nursing (Band 5 status preferred).
  • Preparedness for flexible working (7-day operation support required).
  • An allowance for mileage will be provided.
