Family Practitioner – Disabilities Service (0-18) – Warwickshire County Council
About the Team
An exciting opportunity has arisen for 3 Family Practitioners to join our 0-18 Children With Disabilities service.
This role sits within a supportive, nurturing team dedicated to working with disabled children and their families at all levels of statutory social care support.

About the Role
The Family Practitioners will focus on:
- Conducting 10-day Family Help Assessments.
- Providing longer-term support to vulnerable families.
- Working with disabled children and their families at higher levels of need (Early Help, Child in Need).
- Acting as the Family Help Lead Practitioner.
- Co-ordinating multi-agency collaboration by chairing Child in Need meetings.
- Partnering with professionals across Health, Education, Police, and Social Care to implement effective, family-centred solutions.

- South (Warwick area)
- North (Nuneaton & Bedworth)
- East (Rugby area)

Key Responsibilities
Core Tasks:
- Complete statutory assessments (with supervision, guidance, and support) as the Family Practitioner for:
- Targeted Early Help
- Child in Need plans
- Manage a caseload, including:
- Conducting detailed, legal-compliant assessments of children’s needs (aligned with Warwickshire’s policies).
- Collaborating restoratively with professionals and families to craft effective, ring-fenced support plans.
- Ensuring the child’s voice is front and centre in all decisions and interventions.
- Operate within a context of Safe Uncertainty when working with risks.
- Demonstrate professional curiosity in all interactions (with parents, children, young people, and partners).
Key Strategic Duties:
- Independent multi-agency working—build relationships, facilitate team discussions, and lead child-focused case planning.
- Problem-solving complex family scenarios to find sustainable support solutions.
Requirements
Essential Criteria:
- A qualification in social care (or equivalent).
- At least 3 years’ experience in a social care role, relevant to family support.
- Comprehensive detailed assessments experience, preferably applicable to Early Help and Child in Need forums.
- Proven expertise in direct work with children, young people (0-18), and families:
- Including parenting interventions.
- Managing cases involving resistant or challenging families.
- A collaborative and multi-agency approach capable of balancing independence with team coordination.
- Practical risk assessment skills.
- Fluency in spoken English for daily communication with service users.

Desirable Extras:
- Experience with families who have children with significant disabilities or complex care needs.
- Knowledge of restorative practice frameworks.
