Referral Coordinator (Commission-Based)

About Keyline Home Care

Keyline Home Care helps families receive Medicaid-funded home care services through programs such as Structured Family Caregiving (SFC) and Personal Support Services (PSSX). We're looking for a motivated Referral Coordinator who enjoys following up, building relationships, and helping families move through the enrollment process.

Compensation

  • Commission Only
  • $50 for every case successfully moved
  • 25 moved cases per month is our baseline expectation
  • At our current referral volume, 25 cases is a very achievable target for someone who consistently follows up.
  • There is no cap on your earnings. The more cases you move, the more you earn.

What You'll Do

  • Track all referrals in ----------
  • Contact families and Case Managers promptly
  • Prepare families for phone screenings and nurse assessments
  • Follow up through calls, texts, and emails until the next step is completed
  • Gather required documents
  • Ensure referrals are placed into the correct program
  • Keep cases moving from referral through State Nurse Review
  • Escalate stalled cases when needed
  • Maintain clean and accurate records

Your primary responsibility is simple: keep cases moving.

Performance Expectations

  • Successfully move 25+ cases per month (this is considered very attainable with consistent follow-up)
  • No referral should sit untouched for more than 5 business days
  • Provide weekly updates on stalled cases
  • Maintain strong conversion from referral to phone screening, nurse assessment, and State Review

Who We're Looking For

  • Excellent English communication skills
  • Comfortable speaking with U.S. families over the phone
  • Persistent and proactive with follow-ups
  • Organized and detail-oriented
  • Accountable and self-motivated
  • Experience in healthcare, home care, collections, sales, customer service, or case coordination is a plus, but not required
  • Experience using a CRM or ---------- is preferred
