Our Frailty-ACE
Our Frailty-ACE is an innovative, award-winning approach to supporting frail people with urgent/emergency needs. The co-located Primary Care, and Community ACP is supported by specialist advice from Consultant Geriatricians. It works to support person-centred assessment and care, and to coordinate the services required to support frail patients who would otherwise be admitted or conveyed to remain at home when this aligns with their wishes and/or best interests.
As a Frailty-ACE GP you will work collaboratively, and flexibly with the wider Frailty-ACE to comprehensively assess frail patients' urgent care needs and coordinate holistic person-centred management plans that are in keeping with the patients' preferences and best interests. You will assess and manage the full breadth of clinical presentations and age groups which present in the urgent primary care setting, including physical and mental health needs. As part of the role you will take telephone referrals from Health care professionals within the BNSSG area for appropriate adult medical admissions whilst being sensitive to the capacity issues of the hosting acute trust.
