About the Role

We have an opportunity for you to join the Greater Manchester Assessment and In-reach Team or GMAIC, as it's often referred to. This is a newly created post in a team manager role.

Working closely with the Clinical staff, you will play a key leadership role in ensuring high-quality, patient-centred care, supporting individuals with complex mental health needs in the community.

This is an exciting opportunity to make a real difference by driving service excellence, fostering strong partnerships, and supporting a multidisciplinary team to deliver outstanding outcomes.

Responsibilities

  • Leadership and Care Coordination

    • Work closely with the Clinical staff to ensure high-quality, patient-centred care.
    • Support individuals with complex mental health needs in the community.
  • Service Excellence and Partnerships

    • Drive service excellence.
    • Foster strong partnerships within the multidisciplinary team.
  • Assessment and Management

    • Offer gatekeeping assessments, bed management, and case management to CYP who may require a Tier 4 Inpatient admission.
    • Collaborate with NHS inpatient providers for any Tier 4 referral/request for an Inpatient admission.
  • Crisis Care and Pathways

    • Manage the Single Point of Access for all Inpatient referrals for the GM population of 13-18 year old young people who are being considered for inpatient care.
    • Work closely with external services such as social care, CAMHS, FCAMHS, CMHT, CEDS, EIT, HTT to ensure the correct pathways are in place, to support our CYP’s discharge from hospital.
