HC Care Co-ordinator

At The HICA Group, we pride ourselves on delivering exceptional care and support to individuals across our community. We are looking for a dedicated and proactive Care Coordinator to join our Grimsby Homecare Branch, ensuring high-quality, personalised care services for our clients.
The company is unable to sponsor the employment of international workers in this role. You will be required to provide evidence of your right to work in the UK.
Pay & Hours:
- Rate of pay: £14.00 per hour
- Hours of work: 40 hours per week
- 6 Week Fixed Rota
Duties & Responsibilities:
As a Care Coordinator, you will play a vital role in planning, managing, and delivering exceptional care services. Your key responsibilities will include:
- Planning, developing, and maintaining person-centred and outcome-based services, ensuring rotas are designed for effective service delivery.
- Monitoring recruitment needs within your geographical area and communicating weekly updates to the Registered Manager.
- Managing services in accordance with relevant legislation, ensuring compliance with internal and external quality standards.
- Developing strong relationships with service users, their families/representatives, and other professionals or agencies involved.
- Producing accurate electronic and written records, including daily reports, aligned with company policies and GDPR requirements.
- Participating in the on-call rota to meet operational demands and personally covering care calls in the event of staff shortages.
- Responding to, and reporting, any concerns or complaints in line with company procedures.
- Preparing management reports and providing relevant information as required.
- Liaising with community organisations and stakeholders to strengthen service delivery.

Experience & Qualifications:
We're looking for someone with strong organisational skills, a passion for delivering inclusive care, and the ability to build positive relationships. Preferred experience and qualifications include:
- Previous experience providing support within a care setting, administering medication, and using equipment.
- Basic IT skills, including proficiency in Microsoft Office packages.
- Excellent written and verbal communication abilities.
- Knowledge of risk assessment, support planning, and a person-centred approach.
- A Level 2/3 Diploma in Care or another relevant qualification, such as a Care Certificate (desirable).
Additional Requirements:
- Applicants must have access to their own transport to attend community-based work and will receive a fuel allowance.
- Flexibility is required, as this role involves working some weekends on a rota basis.
