Head of Interface Pathway Development and Change
Shape the Future of Interface Care in Fife
Head of Interface Pathway Development and Change – NHS Fife
This is a unique opportunity to lead one of the most ambitious transformation programmes in Scotland, helping redesign urgent and unscheduled care delivery across acute, community, and health and social care services.
Working directly with the Director of Acute Services and Director of Health and Social Care, you will drive the development of innovative, whole-system pathways, ensuring patients receive the right care, in the right place, at the right time.
About the Role
As a senior leader within NHS Fife, you will spearhead transformational change across a diverse portfolio, including:
- Urgent and Unscheduled Care pathways
- Same Day Emergency Care (SDEC)
- Hospital at Home services
- NHS Fife Virtual Hospital
- OPAT (Outpatient Antimicrobial Therapy) services
- Frailty pathways
- Community interface services
- Flow navigation and admission avoidance programmes
- Virtual wards and remote monitoring models
You will bring together Acute and Health and Social Care Partnership interface services, the Scottish Ambulance Service, and other key partners to deliver integrated solutions that:
- Improve patient flow
- Reduce delays
- Deliver better outcomes for Fife’s population

Key Strategic Priorities
This role will be central to delivering key developments, including:
- Establishing and embedding the NHS Fife Virtual Hospital
- Developing the next phase of the Fife Navigation Centre (FNC+) model
- Expanding Hospital at Home services
- Leading the development of a whole-system escalation and control room function
- Driving pathway redesign across acute, primary, and community services
- Supporting NHS Fife’s wider strategic transformation agenda
Who We Are Looking For
We seek an inspirational leader with the ability to:

Key Requirements
You will have:
- A master’s-level education or equivalent experience
- Significant senior leadership experience within the NHS or similar large public sector organisation
- A strong track record of delivering complex service redesign and transformation programmes
- Experience in:
- Strategic planning
- Programme management
- Pathway development
- Exceptional stakeholder engagement, influencing, and negotiation skills
- Experience of:
- Leading multi-agency collaborations
- Working across organisational boundaries
- Strong analytical and problem-solving skills, with the ability to turn insight into action
