Health and Wellbeing Coach – Temporary Contract (12 Months)

About the Role
A temporary position has arisen to join the Kirklees Personalised Care team.
About the Team
Personalised Care provides individuals with the control and choice over their mental and physical health that they expect in other aspects of life. This programme is a core element of the NHS Long Term Plan, helping people with:
- Long-term illness and complex needs
- Mental health challenges
- Social issues impacting health and wellbeing
It empowers people to make informed decisions about their health and live the life they want to design—aligned with their personal values.
You’ll work within the Personalised Care Team of the Mast Primary Network (PCN). This network covers five GP practices in Kirklees, including Scissett, Denby Dale, Skelmanthorpe, Shepley, Lepton, Kirkheaton, and Kirkburton, working collaboratively to improve local healthcare provision alongside NHS, local authority, and voluntary sector partners.

As a Health and Wellbeing Coach (HWBC), your responsibilities include:
- Using health coaching techniques to support individuals in developing self-management skills, enabling them to:
- Gain confidence in managing their health
- Set and achieve health and wellbeing goals
- Enhance lifestyle changes
- Providing access to self-management education, peer support, and social prescribing
- Targeting proactive preventive care to help individuals identify and manage existing health issues before they worsen
- Working alongside clients through multiple coaching sessions to:
- Assess health needs
- Set personalised goals
- Implement actionable health and wellbeing plans
Due to the nature of the role, travel between locations within the Mast PCN footprint (Huddersfield and surrounding areas) is essential. Access to transportation is required, and flexibility to work outside regular hours (including evenings and weekends when needed) may be necessary.

Huddersfield & surrounding areas, covering Mast PCN areas.
Contract Type & Duration
- Position: Permanent (but currently advertised as temporary)
- Length of Contract: Fixed term for up to 12 months
- Hours: 37 hours per week (Monday–Friday)
- Salary: £32,061–£33,699 (Grade 8)
Additional Requirements
- An Enhanced Disclosure and Barring Service (DBS) check must be completed
- A conviction could impact recruitment decisions but will be considered on an individual basis
