Health and Wellbeing Practitioner: HMP Send (part time)
Location: Surrey
Salary: A GBP 13,928.57 per annum
Vacancy Type: Permanent
About The Role
The Forward Trust is seeking a compassionate and motivated Health and Wellbeing Practitioner to join our team at HMP Send, Surrey, working 18.75 hours per week. As part of our integrated Health and Wellbeing Service, you'll support women experiencing substance misuse and low-level mental health challenges, helping them build healthier, more positive futures within a trauma-informed and gender-responsive environment.
Working as part of a multi-disciplinary team, you'll deliver recovery-focused interventions, provide practical support, and help service users prepare for successful reintegration into the community.
What You'll Be Doing
- Managing a caseload and coordinating individual support plans
- Conducting assessments and developing person-centred care plans
- Delivering structured one-to-one sessions and facilitating group programmes
- Providing harm reduction advice and guidance
- Supporting release planning and through-care arrangements
- Working closely with healthcare, mental health, and prison colleagues
- Maintaining accurate records and contributing to service performance targets
- Undertaking second signatory duties and oral swab testing where required

What We're Looking For
- Experience supporting individuals with substance misuse, mental health, or complex needs
- Excellent communication and relationship-building skills
- A proactive, organised, and solution-focused approach
- Confidence working within a multi-disciplinary environment
- Strong IT and record-keeping skills
- Commitment to completing the Drug and Alcohol Workforce Development Programme (DAWFDP)

All prison-based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 5 months to complete. Any offer of employment will be conditional upon the successful completion of both checks.
