About the Role

Forward Trust is seeking a passionate and motivated Health and Wellbeing Practitioner to join their Connections Team at HMP Millscare.

This is a full-time post (35 hours per week) offering a unique opportunity to support prisoners in improving their physical, mental, and emotional wellbeing, building resilience, and fostering healthier lifestyles that can be sustained post-release.

You’ll work collaboratively with prison staff, healthcare professionals, peer mentors, and external partners to create a positive and recovery-focused environment.


Responsibilities

As a Health and Wellbeing Practitioner, your core duties will include:

  • Delivering 1:1 support, group interventions, and wellbeing workshops
  • Coaching and mentoring service users to achieve sustainable, positive change
  • Supporting and developing Prisoner Health and Wellbeing Champions
  • Promoting healthy lifestyles, recovery, and wellbeing across the prison
  • Organising health promotion events and awareness campaigns
  • Building strong partnerships with healthcare, mental health, and substance misuse services
  • Supporting the delivery of a varied programme of wellbeing activities, including evenings and weekends
  • Maintaining accurate records, care plans, and case management documentation

  • Has strong interpersonal skills, confident in building positive relationships and motivating others
  • Can deliver both 1:1 and group-based interventions effectively
  • Is organised, proactive, and capable of managing a varied caseload
  • Thrives in a multidisciplinary environment
  • Is dedicated to safeguarding, inclusion, and person-centred support
