About the Role
Forward Trust is seeking a passionate and motivated Health and Wellbeing Practitioner to join their Connections Team at HMP Millscare.
This is a full-time post (35 hours per week) offering a unique opportunity to support prisoners in improving their physical, mental, and emotional wellbeing, building resilience, and fostering healthier lifestyles that can be sustained post-release.
You’ll work collaboratively with prison staff, healthcare professionals, peer mentors, and external partners to create a positive and recovery-focused environment.
Responsibilities
As a Health and Wellbeing Practitioner, your core duties will include:
- Delivering 1:1 support, group interventions, and wellbeing workshops
- Coaching and mentoring service users to achieve sustainable, positive change
- Supporting and developing Prisoner Health and Wellbeing Champions
- Promoting healthy lifestyles, recovery, and wellbeing across the prison
- Organising health promotion events and awareness campaigns
- Building strong partnerships with healthcare, mental health, and substance misuse services
- Supporting the delivery of a varied programme of wellbeing activities, including evenings and weekends
- Maintaining accurate records, care plans, and case management documentation

Requirements
We are looking for someone who:
- Is passionate about health, wellbeing, and recovery
- Has strong interpersonal skills, confident in building positive relationships and motivating others
- Can deliver both 1:1 and group-based interventions effectively
- Is organised, proactive, and capable of managing a varied caseload
- Thrives in a multidisciplinary environment
- Is dedicated to safeguarding, inclusion, and person-centred support
