We are looking for a compassionate and motivated Health Care Support Worker to join our Community Hospital team.

In this rewarding role, you will work alongside nurses, therapists, and other healthcare professionals to provide high-quality, person-centred care and support patients with their rehabilitation and recovery. You will assist with the assessment, planning, delivery, and evaluation of care programmes, helping patients achieve their goals and maintain independence. This is an excellent opportunity for someone who wants to start or develop a career in Healthcare and is passionate about making a positive difference to patients' lives while working within a supportive multidisciplinary team.

Key Responsibilities

  • To be responsible for providing accurate records of information required by the Trust for audit purposes.
  • To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register.
  • To give holistic care to patients in the community Inpatient setting.
  • To identify and report any changes in patient care needs or changes in physical or emotional well-being to the Ward Manager. To report any changes to the Senior Nurse.
  • To maintain accurate records in the RiO electronically, record new problems as they are identified, and report all actions taken to the Senior Nurse.
  • To act as the patient’s advocate and, by providing information and support, facilitate the patients' own choices with regard to personal care, promoting independence and self-care as appropriate.
  • To be skilled in communication, establishing and maintaining good relationship with patients, carers, members of the MDT Team, and other agencies involved, making referrals, and reporting these actions to the team lead.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training, and the allocation of equipment. As part of the process, new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing, and key processes. The induction will be held at our head office in Goodmayes Hospital.

Probationary Period

This post will be subject to a probationary period.

High Cost Area Supplement - Outer London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,870 and a maximum payment of £6,137 per annum, pro rata for part-time).
