Coventry and Rugby Community Learning Disability Team
The Coventry and Rugby Community Learning Disability Team has an exciting opportunity to join the team. The role will primarily be working in health facilitation, supporting people with a learning disability both to avoid acute hospital admissions and to support with complex discharges. This will involve:
- Jointly working with the acute Hospital L/D liaison Nurse, local GP's community services, alongside Community learning disability Nursing roles.
- Promoting the needs and rights of people with learning disabilities to other agencies and the general public.
- Identifying and evaluating specific client needs through specialist assessment.
- Assisting in the development of new services and projects within the community to meet identified client’s needs, including those with complex needs and/or behaviours that challenge.
- Participating as an active member of the Learning Disability Nursing Team and establishing and maintaining effective communication with other agencies and professionals.
- Acting as a health facilitator for people with a learning disability both to avoid acute hospital admissions and to support with complex discharges, jointly working with the acute Hospital L/D liaison Nurse.
