Overview
As a strategic lead, you will shape and drive continuity of care pathways ensuring individuals released from prison access vital treatment and support, reducing reoffending rates. Accountable to the Head of Community Integration, this role is central to health, substance misuse, and resettlement strategies across the Probation Service (PS).
Opportunities are available locally with the expectation for inter office travel and regular prison visits.

Key Responsibilities
Your role will focus on strengthening continuity of care through partnership working around substance misuse and health-related release planning. This involves:
- Consultation and policy development to improve data sharing and support informing release conditions.
- Collaboration with HMPPS, health providers (including NHS Reconnect), and local community stakeholders.
- Ensuring adherence to HM Prison Service (HMPPS) drug strategy, integrating health and probation services.
Confidentiality and sensitive information handling guidelines must be followed strictly.
Responsibilities, Activities And Duties
The role involves:
Leadership & Partnerships
- Foster relationships with prisons, treatment providers, and community stakeholders to streamline effective exit from care pathways.
- Lead regional collaboration to improve substance misuse and health outcomes for offenders post-release.
- Represent regionalised probation and prison services (including Offender Health) at national and local levels.

- Use performance analytics to refine service delivery.
- Proactively identify innovation and good practice improvements and share them at national levels.

- Be the key local point of contact for regional performance, challenges, and national intervention strategies.
Risk & Impact Driven Action
- Provide specialist advice on managing the transition from prison to community, supporting prison drug strategy leads and Probation Guidance Document (PGD) leads.

Requirements & Experience
Essential
- Proven criminal justice or prisoner engagement skills, emphasising outcomes-based continuity of care pathways.
- Broad experience working across government departments, health, and criminal justice organisations.
- Influencing capabilities, strategic conflict resolution, and action-focused goal achievement.
- Providers of Large-Scale Transformation — experience demonstrating deliverable change and improved system efficiency.
- IT literacy with performance data interpretation ability.
- Commitment to diversity, innovative practice, and engaging effectively with vulnerable groups.
- Relevant health/mental health/social care pathways knowledge, particularly post-custody resettlement.
