About the Role
As a Healthcare Assistant, you will join our valued Gravesham Central Community Team based at Gravesham Community Hospital. This role provides a rewarding opportunity to deliver care where it matters most—right in people’s homes and local clinics, helping individuals maintain their independence and receive continued support in their communities.
This position is ideal for someone passionate about working in community-based care rather than in a single fixed location all day. You will forge meaningful relationships with patients while playing a critical role in supporting their health and well-being in a personalised, one-on-one environment. As part of a supportive, close-knit community nursing team, you will collaborate to deliver high-quality, compassionate care across the Gravesend area.

Main Responsibilities
This community-based role involves providing vital nursing care to patients in their homes and local clinics, ensuring a varied and dynamic position where no two days are alike. Your contributions will directly impact patients’ ability to manage their health, recover from illness, and maintain independence.
Key responsibilities include:
- Assisting patients with delegated nursing care in their own homes, adhering to clinical guidance
- Carrying out clinically trained tasks, such as dressings and glucose monitoring, after training and competency verification
- Supporting holistic and evidence-based care, tailored to each patient’s unique needs and circumstances
- Maintaining accurate and up-to-date patient records via electronic systems
- Lending administrative support to the broader nursing team as needed
- Planning and organising your daily visits independently across the community to optimise efficiency and coverage
Essential Travel Requirement: This role involves travel across the local Gravesend area, so a valid UK driving licence and access to a reliable car are mandatory.
Person Specification
Essential Criteria
- NVQ Level 2 or 3 in Health and Social Care, or equivalent relevant experience
- A genuine passion for and interest in community-based care, with an enthusiasm for supporting patients in their home environments
- A valid UK driving licence and access to a vehicle
- Excellent communication and interpersonal skills, with the ability to build trustful relationships with diverse patient groups
- Independence and strong self-management skills to efficiently organise work schedules and meet professional deadlines
- Basic proficiency in IT, including proficiency with Microsoft Office applications
Desirable Criteria
- Previous experience in community-based or domiciliary care settings
- Demonstrated care experience supporting a broad range of patient needs outside traditional clinical environments
- Autonomy and confidence in working effectively and safely solo in the community
