Transitional Care Liaison – Ohio Living

About the Role
We are seeking a Transitional Care Liaison to conduct and supervise the long-term care admissions process, ensuring we achieve and exceed occupancy goals for our life plan communities in compliance with company, federal, state, and local standards.
Responsibilities
The role encompasses four primary areas:
Administration & Reporting (50%)
- Exercises discretion and independent judgment on significant admissions matters
- Facilitates pre-certifications and re-certifications for insurance patients
- Serves as the initial contact for the majority of incoming patient records
- Collects detailed information for referral management and performance statistics
- Maintains admissions and referral records
- Shares observations to improve departmental quality and efficiencies
- Attends all-employee and departmental meetings, in-service training
Sales & Referral Management (20%)
- Expands referral sources by informing them of services, strengths, and community viability
- Networks at events (may include early mornings or evenings)
- Handles unfillable referrals appropriately
- Visits hospitals to meet prospective residents
- Manages communication between clinical teams and insurance companies
- Conducts community tours
- Protects confidentiality of patientresident information

Reimbursement Management (10%)
- Provides billing documentation (pre-certifications) to Corporate Finance
- Assesses payment ability and cost containment for potential residents
- Proactively verifies updated resident charts and nursing notes for insurance discussions
- Ensures signed contracts and admissions paperwork upon admission
Relationship & Community Management (10%)
- Fosters positive relationships with residents, families, staff, and third-party payers
- Provides clear, timely communication to all stakeholders
People Management & Development (10%)
- Oversees adherence to Employee Handbook requirements
- Addresses compliance gaps with employee feedback and HR as needed
- Develops, schedules, and monitors in-service and onboarding training
Note: Additional duties may be assigned.
Requirements
Education
- Associate degree in a related field (required)
- Bachelor’s degree in marketing or a related field preferred
- Preferred: Current unencumbered RN or LPN license in Ohio
Experience
- Three+ years in long-term care, hospital++, or clinical settings (required)
- Two+ years in long-term care admissions preferred
- Preferred: Sales and marketing experience

Skills & Competencies
- Proficient in: Windows, Microsoft Office (Word, Excel, PowerPoint), internet
- Language fluency: Full literacy in English for written/speaking tasks
- Licensing: Valid driver’s license with an acceptable driving record
- Insurability for professional liability & crime coverage
Working Conditions & Requirements

Physical demands: Up to 6 hours/day sitting/standing/walking; lifting up to 25 lbs.

Travel demands: Up to 6/hour driving, occasional weekends/holidays/on-call shifts

Exposure: Minimal patient interaction with disease processes; possible exposure to infectious waste (Hepatitis B, HIV/AIDS risk category: II)
