Role Description
This full-time, on-site Healthcare Worker role is based in Redditch and involves providing direct care and support to individuals in line with their care plans. Day-to-day tasks include:
- Assisting with personal care
- Supporting daily living activities
- Monitoring wellbeing
- Reporting any changes in health or behavior to appropriate colleagues

The role includes promoting independence, dignity, and choice, as well as supporting community participation and access to relevant health and social services. The Healthcare Worker will:

- Maintain accurate and timely documentation
- Follow company policies and procedures
- Collaborate with multidisciplinary teams, families, and external agencies to ensure consistent, high-quality care
The position also requires adherence to safeguarding, infection control, and health and safety standards at all times.
Qualifications
- Ability to deliver effective Community Health support, including awareness of local services and resources.
- Strong Communication skills, with the ability to interact clearly and respectfully with service users, families, and colleagues.
- Competence in Health Education and Health Promotion to help individuals understand and manage their own health needs.
- Experience or training in Case Management, including coordinating care, signposting, and follow-up actions.
- Relevant care-related qualification (e.g., NVQ/QCF in Health and Social Care or equivalent) or willingness to work toward one.
- Knowledge of safeguarding, person-centered care, and confidentiality requirements within health and social care settings.
- Ability to work flexibly, including evenings and weekends if required, and to handle physically and emotionally demanding situations.
- Previous experience in a healthcare, community care, or social care role is desirable but not essential; full training may be provided.
