Role Description

This full-time, on-site Healthcare Worker role is based in Redditch and involves providing direct care and support to individuals in line with their care plans. Day-to-day tasks include:

  • Assisting with personal care
  • Supporting daily living activities
  • Monitoring wellbeing
  • Reporting any changes in health or behavior to appropriate colleagues

  • Maintain accurate and timely documentation
  • Follow company policies and procedures
  • Collaborate with multidisciplinary teams, families, and external agencies to ensure consistent, high-quality care

The position also requires adherence to safeguarding, infection control, and health and safety standards at all times.

Qualifications

  • Ability to deliver effective Community Health support, including awareness of local services and resources.
  • Strong Communication skills, with the ability to interact clearly and respectfully with service users, families, and colleagues.
  • Competence in Health Education and Health Promotion to help individuals understand and manage their own health needs.
  • Experience or training in Case Management, including coordinating care, signposting, and follow-up actions.
  • Relevant care-related qualification (e.g., NVQ/QCF in Health and Social Care or equivalent) or willingness to work toward one.
  • Knowledge of safeguarding, person-centered care, and confidentiality requirements within health and social care settings.
  • Ability to work flexibly, including evenings and weekends if required, and to handle physically and emotionally demanding situations.
  • Previous experience in a healthcare, community care, or social care role is desirable but not essential; full training may be provided.
