### [About Your Role]
You’ll play a vital role in managing and delivering sustainable Healthy Communities projects and programmes across a local contract area, ensuring measurable impact for the community, with a particular focus on tackling health inequalities.
Key Responsibilities:
- Plan, implement, and oversee Healthy Community projects/programmes (such as Exercise Referral schemes, gym-based classes, or falls prevention initiatives) in alignment with approved parameters.
- Follow relevant training/qualifications to deliver approved programmes independently.
- Complete monitoring and evaluation tasks as requested by your line manager in line with current data protection protocols.
- Collaborate with stakeholders to ensure projects align with community needs and impact targets.

### [About You]
We’re looking for an individual with a genuine passion for improving local community health. Please ensure you meet the following criteria:
- A qualified instructor (e.g., Gym Level 3, NGB Level 2+, or equivalent) capable of delivering sessions independently.
- Experience in planning, delivering, and monitoring health-related programmes/sessions.
- Proficiency and comfortable using Microsoft Office tools (MS tools).
- A strong desire to make an impact in improving community health and wellbeing.
