We are seeking an experienced and motivated Heart Failure Specialist Nurse to join our established Heart Failure Service on a fixed-term maternity leave basis.

This role is ideally suited to a clinician who is already working within a specialist heart failure service and can contribute rapidly to the delivery of high-quality care across acute, community, and outpatient settings.

The successful candidate will manage a specialist caseload of patients with heart failure, undertake autonomous clinical assessment, lead nurse-led clinics, optimise guideline-directed medical therapy and work collaboratively with the wider multidisciplinary team to deliver excellent patient-centred care.

Applicants should have substantial experience in the management of heart failure and the delivery of specialist heart failure services. An Independent Non-Medical Prescribing qualification is strongly preferred and may be considered essential depending on service requirements.

This is a full-time post; however, we are committed to supporting flexible working and welcome applications from candidates seeking alternative working patterns or reduced hours.

  • Provide clinical care for a caseload of patients using advanced specialist skills and knowledge to undertake full assessments of patients with chronic heart failure.
  • Provide support to patients, families and carers ensuring clear communication, and accurate information in relation to the diagnosis of heart failure, treatment and the maintenance of a healthy lifestyle, promoting self-management, and optimising quality of life.
  • Accountable for own professional actions and clinical decisions, acting as an advanced independent practitioner/decision maker.
  • Ensure the efficient delivery of a specialist heart failure service, responding to changes in demand, being proactive in-service review.
  • Work in collaboration with multi-disciplinary teams and colleagues in both hospital and community setting.
  • Provide palliative care and end of life support to patients with advanced heart failure.
  • Act as a resource promoting evidence-based practice and best management of heart failure across inpatient and community services.
  • Participate in delivering structured education to other healthcare professionals.
  • Participation in the National Heart Failure Audit.
