Highly Specialist Physiotherapist
Exciting Opportunity for a Physiotherapist
An exciting opportunity has arisen for an enthusiastic and innovative Physiotherapist to work within the Falkirk Health and Social Care Partnerships ReACH Service. The ReACH Teams provide multidisciplinary assessment and rehabilitation for adults in the community.
Role Overview
The role encompasses Physiotherapy assessment, diagnosis, and treatment of patients within the community as part of a multidisciplinary team. There will be a focus on preventing unnecessary hospital admission and supporting earlier hospital discharge in line with the national Discharge without Delay programme.
Key Responsibilities
- Delivering highly skilled physiotherapy to patients in the community.
- Acting as a proactive member of the multidisciplinary team.
- Serving as a senior clinical decision maker, leading complex care and delivering rehabilitation.
- Making onward referrals to other services as required.
- Supporting the development of less experienced and unregistered staff within the team.
- Contributing to practice education and service improvement.
Requirements
- Broad previous experience with highly developed skills in community rehabilitation, or frailty and falls prevention.
- Proven record of service development.
Additional Information
- The successful candidate will be based in Falkirk Community Hospital with the ReACH Falkirk Team.
- The post involves weekend working as the ReACH team provides a 7-day service.
- The successful candidate will act as a Forth Valley wide resource for advice on treatment and management of complex patients in conjunction with other Highly Specialist AHP’s in the ReACH Teams.

Protecting Vulnerable Groups (PVG) Scheme
The duties of this post require the successful candidate to be a member of the Protecting Vulnerable Groups (PVG) Scheme. More information on this scheme can be found at www.disclosurescotland.co.uk.
Working Hours and Pay
As from 1/4/26, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part-time hours will be reduced pro-rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
