Job Overview
Join a passionate and forward-thinking HIV Multidisciplinary Team where you’ll play a vital role in empowering people living with HIV to take control of their health and wellbeing. Working independently across the community, you’ll build meaningful one-to-one relationships with clients, using motivational interviewing and health coaching techniques to encourage engagement with HIV care, improve adherence to medication, and support broader health outcomes.
As a key member of the wider multidisciplinary team, you’ll collaborate closely with colleagues and support the Clinical Operations Lead in delivering the aims and objectives of the service, Directorate and Trust. The role requires flexibility and adaptability, with a willingness to work across the community, including domiciliary visits, and varying days and hours to meet the needs of the service and the individuals we support.
Main duties of the job
- Work largely autonomously, managing own time and caseload.
- Liaise closely with Sexual Health & HIV service multidisciplinary team (MDT) to provide a coordinated package of care to clients.
- Establish/maintain a network of referral and support agencies to support coordinated care.
- Fully assess the social and emotional needs of people living with HIV and develop appropriate packages of care to meet individual needs holistically.
- Liaise effectively with social services, primary care, local council and community partners to ensure appropriately coordinated care/support is available to facilitate engagement in HIV care as an outpatient.
- Provide practical support within the community to support clients to engage in HIV care, following a management plan agreed by the HIV MDT to include clinical interventions including phlebotomy.
- Provide one-to-one or telephone support using a motivational/health coaching approach to empower clients and achieve positive outcomes.
- Domiciliary/community visits including venepuncture for HIV monitoring blood tests to facilitate ongoing engagement for those who are unable to travel to clinic for care, within the framework of the lone worker and departmental guidelines.
- Support clients to attend healthcare appointments, including transporting them to appointments where required.
- Provide advice and support with applying for benefits, housing, asylum or visas

Detailed Job Description And Main Responsibilities
Health Education/Training
- Participate in HIV promotion work/training across the local health economy to raise awareness and improve understanding/quality of care for this client group.
- Attend regular clinical supervision.
- Responsible for updating own professional knowledge base and take an active part in own professional development.
Professional
- Work independently and in effective liaison with the wider MDT (HIV MDT at Luton Sexual Health and HIV team at iCASH Bedford).
- Identify Safeguarding concerns and follow reporting/management protocols
- Maintain up to date knowledge and awareness of developments in sexual health and related subjects.
- Demonstrate a committed, proactive approach to own appraisal and Personal Development Plan.
- Be a positive role model within the service.
- Represent the service at meetings/events as appropriate.
- Record accurately/concisely all client interventions and interactions in patient record systems
- Maintain an accurate client data base.
- Perform relevant admin tasks as service requires e.g. write letters and reports.
- Ensure that an effective audit system is established and maintained.
- Produce publicity for HIV Community Services e.g. leaflets and posters.
- Report own sickness, annual leave and other absences to line manager in accordance with Departmental and Trust policies.
- Collection of data / statistics required for research purposes.
