Rainbow Care Solutions

Rainbow Care Solutions is a long-established, CQC-registered home care provider with over 20 years experience. We're recruiting Full-Time Home Care Assistants (37.5 hours/week) to support our Discharge to Assess (D2A) / Reablement service across Herefordshire.

This role supports people returning home from hospital to recover safely and regain independence doing with, not doing for.

Location

Herefordshire (community-based)

What you'll be doing

  • Supporting people at home after discharge with reablement-focused care
  • Assisting with personal care where needed, while encouraging independence
  • Supporting daily living skills (washing, dressing, meal prep, mobility, routines)
  • Following care plans and recording notes accurately
  • Reporting changes in condition, concerns, or risks promptly
  • Working respectfully with families and professionals as part of a wider team

What we're looking for

  • Caring, reliable and professional approach
  • Experience in home care is helpful, but not essential (full training provided)
  • Good communication and respect for dignity, choice, and independence
  • Full UK driving licence and access to a vehicle (essential for Herefordshire)
  • Right to work in the UK
