Salary: FC7 £34,914.86 - £43,437.09

Working Pattern: 36 hours per week (Dayshift - Backshift rotation on a 4 on 4 off rota).

Job Details

To provide a Care at Home/Falls Response Service to vulnerable people in the community.

You will work as a Home Care Coordinator within the Home Care Associated Services Team, which is part of Fife Health and Social Care Partnership.

The role will include:

  • Managing team performance, organising supervision, 6 monthly direct observations and direct observation of the people in your team.
  • Coordinate response/service to Community Alarm activations, out of hours social work teams, short terms support and Fife Falls.
  • Responsible for Fife Council Care at Home out with office hours, supporting staff, service users, external providers and staff.
  • Ensuring staff have the appropriate training and skills to deliver quality and safe care, providing support on site with training, demonstration and advice.
  • Providing guidance and instruction to enable staff to practice safely and effectively.
  • Manage staff absence as per local procedures.
  • Working with Health and Social Care Partnership colleagues and colleagues within partnership agencies.

The Person

  • SVQ 3 in Health and Social Care or willing to work towards.
  • Ability to undertake shift working.
  • Previous Care at Home experience.
  • Awareness of the Health and Social Care Standards. 
  • Leadership skills.

Before confirming to your appointment, you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland and become a member of the relevant PVG scheme.

As this role requires SSSC registration, you will have three months to apply for registration.

If appointed, you must start your application as soon as possible so you're registered within the required six months.

If you need to achieve a qualification to maintain your SSSC membership, information will be provided if you are appointed. You must ensure you complete the qualification within the specified timeframe.
