Homecare Care Coordinator (Maternity Cover until March 2027) - Tower Hamlets

£30k Pro Rata Depending On Experience

We have a fabulous opportunity for a Care Coordinator to join our well-established Homecare Service covering the vibrant Tower Hamlets area of East London.

You will need to have experience within a Home Care setting - ideally as a Care Co-ordinator, however if you have experience as a Home Care worker or Senior Care worker and are passionate about providing the highest quality care to our service users and are ready for the next step then this could be the right fit for you.

What you can expect:

  • Salary: Up to £30k (depending on experience)
  • Working Hours: 8:30am - 5:00pm, Monday to Friday, plus on call as per the rota every 6 or 7 weeks.

Main responsibilities as Homecare Care Coordinator:

  • Proactively managing new care package referrals
  • The allocation of Homecare care workers' rota's and workloads on the electronic management system
  • Matching care workers to service users based on care need, skills, experience, and personalities.
  • To maintain effective links and communication with local authority teams, care workers, service users, relatives, and other relevant stakeholders.
  • Following up on any concerns raised by care workers and liaising/referring to appropriate professionals, including GP's, OT's, and DN's
  • Ensuring care workers are appropriately trained and able to carry out their duties and responsibilities effectively and sensitively.
  • Maintaining efficient administrative systems and records.
  • To implement and monitor all care packages and care plans as detailed within individual service agreements
  • To record and report any complaints whilst ensuring that all quality standards, complaints, and comments are dealt with promptly.
  • To deal with emergencies arising from the service
  • To manage the homecare service and staff in accordance with the organisation's aims and objectives, policies, and procedures.
  • To assist in reviews of all Homecare care worker inductions and training needs on a regular basis to meet the annual training requirements.
  • To monitor and record absence for allocated staff.
  • To carry out regular supervision and monitoring of Homecare care workers in line with the service's registration requirements.
  • To provide an emergency 'On Call' service as per rota
  • To undertake quality assurance processes, including telephone monitoring calls and care record audits

About you:

  • Previous experience working as a Care Coordinator is a desirable requirement or considerable experience within Home Care.
  • Understanding of CQC regulations and requirements.
  • Excellent IT skills and competency in Microsoft Office packages as well as databases or people rostering systems.
  • Possess excellent verbal and written communication skills with an eye for detail ensuring accuracy of data.
  • Ability to work under pressure with changing demands that require you to be responsive and flexible.
  • Excellent people skills with the ability to provide an exceptional level of service.
  • Experience of prioritising tasks and meeting deadlines without compromising accuracy
