Homeless Recovery Coordinator

Overview A fantastic, new opportunity has arisen to join our team and become a Homelessness Recovery Co-ordinator, supporting rough sleeping individuals throughout their treatment journey. We have an exciting opportunity for a Homeless Recovery Coordinator who has the skills and experience to assess and engage with our service users, ensuring that they access the right intervention at the right time, in a way that best meets their needs. Where: Birmingham Hours: Full time, 37.5 per week Contract Type: Permanent *Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours. If you have recently applied, need not apply again Responsibilities About the role: Establishing effective communication and referral pathways those individuals who are homeless and in contact with other health and care agencies Regular street outreach, hospital visits and working at hostels providing drop in services Work closely with the local authority, Health & Social care sector, partners and any provision linked to Homelessness and improve joint working, attending multi-disciplinary meetings when as and when required Supporting service users from point of entry into the service and through their treatment/recovery journey Providing screening, assessment, and recovery planning and onward referral Coordinate the transfer of any accommodated out of area individual. This may include referral to and liaison with their new drug and alcohol service and pharmacy and provision of bridging prescription until an appointment and prescription is provided. Promoting carer, service user and community involvement Providing advocacy for access to partnership services Working with service users to support social (re)integration, enabling them to lead meaningful and purposeful lives: promoting recovery, resilience, peer support and self-determination About you: You will focus on engaging with people who have a history of rough sleeping and substance misuse. You will have some experience of engaging with individuals with multiple and/or complex needs and demonstrate a determined approach to finding solutions. Some knowledge of one or more of the following is required: Homelessness Mental health Criminal Justice System Substance misuse Have a strong knowledge and understanding of substance misuse issues Experience of engaging with a wide variety of individuals with a variety of backgrounds and needs Be an excellent communicator, both verbal and written with good IT skills Proactive and able to work on own initiative with excellent time management and prioritising skills Have a good working knowledge of mental health interventions, services, and good practice Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders Ability to manage change successfully in a way that prioritises the needs of service users Understand the importance of information governance processes and commit to follow and apply all necessary safeguards What we will give to you: 25 days holiday (+ bank holidays) rising by 1 day for each years’ service “Capped at 30 days” Paid ‘Wellness’ hour each week along with a ‘Wellness’ hub and Employee Assist Programme Contributory pension scheme Several benefits incl. discounts for shopping, cinema, holidays, etc. A friendly and supportive team Training, career development & progression opportunities Direct applications only — we will not be engaging agencies for this vacancy.
