Location: 590 Seven Sisters Road, London, N15 6HR (with additional work at North Middlesex University Hospital) Working Hours: 22.5 hours per week (includes evening and weekend shifts) Contract Type: Permanent Salary: £28,690–£34,730 FTE (pro-rated for part-time hours, starting pay dependent on experience and progression)

About the Role
As a Hospital Link Worker, you’ll work in Haringey, London, bridging the gap between hospital-based care and community-based treatment for individuals with alcohol or drug-related concerns. You’ll split your time between our office (590 Seven Sisters Road) and wards at North Middlesex University Hospital, proactively identifying and assisting high-intensity users who frequently access hospital services.
Key Responsibilities
- Deliver 1:1 psychosocial interventions in hospital wards and Emergency Departments (ED), engaging patients in drug and alcohol treatment services (specifically within Haringey).
- Conduct initial screenings and triage assessments for new patients, while offering crisis intervention and rotatory duty cover.
- Collaborate with multi-agency partners, including (but not limited to):
- Mental health services
- Primary care teams
- Disability units
- Social care providers
- Monitor and improve outcomes to reduce preventable GP, ED, and A&E attendances, ambulance call-outs, and hospital admissions for frequent attender cohorts.
- Carry out ward rounds to identify patients with alcohol-specific needs and facilitate entry into appropriate treatment pathways.
- Build trust and rapport with frequent hospital attendees by making regular, proactive contact (via homes, clinics, or out-and-about interventions) using motivational interviewing techniques.
- Advocate for housing, benefits, mental/physical health, and tailored drug/alcohol treatment, according to service user needs.
- Empower service users to develop self-esteem, social skills, and psychological resilience as part of their recovery from addiction.

Requirements
- Professional experience in:
- Health and social care
- Community work
- Hospital or acute settings
- Familiarity with motivational interviewing (training can be provided for suitability baselines)
- Solid understanding of substance misuse (alcohol/drugs), including recovery pathways
- A proactive, empathetic approach—armed with flexible resilience to engage with complex, challenging individuals
- Ability to work independently, showcase empathy, and advocate effectively within a multi-agency environment
